Health TRUST Rockdale Grant Evaluation Report Forms
Grantees are required to submit evaluation reports describing progress that has been made toward achieving the original project goals as stated in their grant applications. Interim Evaluation Reports are required for grant-funded projects/programs with implementation timelines of six months or longer. Final Evaluation Reports are required for all grants. Grantees should refer to their grant award letter for the due dates of their interim and final reports.
